Did you know?

As a full-time registered Durham College/UOIT student you areautomatically covered by the Student Insurance Plan. Full-time Trent inOshawa students may opt-in to the plan as well. Student’s may alsochoose to opt-in their spouse and dependent children.

Pharmacy on campus

  • Vision Care
  • Orthopedic supplies
  • Prosthetic appliances
  • Clinical psychologist or Speech therapist (if recommended by doctor)
  • Naturopath or Chiropractor
  • Physiotherapist (if recommended by doctor)
  • Massage therapy (if recommended by a doctor)
  • Medical supplies

Drug Benefits

Drug benefits cover 80 per cent of the cost of most prescriptionsdrugs, including all vaccines (immunizations) up to a maximum of $3000for one policy year from September 1 to August 31. The dispensing feecap is $10 for prescriptions.

Dental benefits

Dental benefits cover 100 per cent of one examination andconsultation, necessary x-rays and diagnostic services during eachpolicy year.

Also included:

  • 100 per cent of one cleaning
  • One unit of polishing
  • 75 per cent of the cost of fillings space maintainers
  • 75 per cent of the cost of two wisdom teeth extractions

Coordination of benefits

Students who have existing insurance coverage can increase theircoverage through a coordination of benefits. This means that when aclaim is made priority payment is established as follows:

1. The plan where the insured is covered as a student.

2. The plan where the insured is covered as a dependent.

Can I track my own Claims? Can you reimburse my claim using direct deposit?

Yes. Once registered, plan members/dependents can:

  • View personal claims history;
  • Select direct deposit for claim pay¬ments;
  • Access dependent claims information (for those individuals under the age of majority);
  • Obtain details on the reason for particular claim adjustments or rejections;
  • Submit coverage queries online – “Ask the Expert;”
  • Print individual claims for coordi¬nation of benefits (COB);
  • Run consolidated statements for tax purposes;
  • Access claim forms and important health information.

Register online by visiting www.claimsecure.com. Click on the “member” key located under the “online services” menu, and follow the on-screen guide.

Opt out and opt in process

Opt out: Students may decline coverage for either or both plans if theyhave existing coverage, by going to www.aclstudentbenefits.com andcompleting the online opt out form. You will need the name of theinsurance company under which you are covered, the policy number andsubscriber name. Please refer to opt out deadlines.

Opt in: Students who stay on the Student Health Insurance Plan mayopt in dependents (spouse/children) by completing the opt in form inYour SA office. A certified cheque or money order is required forpayment. We do not accept cash or personal cheques. Students can alsoopt in online at www.aclstudentbenefits.com with a VISA. Please referto the deadlines.

Deadline September 30, 2009

January-start students:

Begins January 1, 2010

Deadline January 31, 2010

February-start students:

Begins February 1, 2010

Deadline February 28, 2010

May-start students:

May-start students must complete a paper opt out form through Your SA office and provide proof of coverage. NO EXCEPTIONS made if the deadline is missed.

REFUND CHEQUES MUST BE PICKED UP WITHIN SIX MONTHS OF OPTING OUT. STALE-DATED CHEQUES WILL NOT BE REISSUED. CHEQUES ARE ISSUED IN THE NAME OF THE STUDENT AND A CURRENT CAMPUS ID CARD MUST BE PRESENTED TO RECEIVE A CHEQUE. STUDENTS WILL NOT BE CONTACTED INDIVIDUALLY OR THROUGH MAIL SERVICE.
